Notification of removal of the MT5 MAMM manager API Call

This is advanced notification of the intended removal of the mt5_mamm API call. The API call is not currently being used and will removed the week starting the 4th of November 2019.
As part of this change the related attributes mamm_status and manager_id will also be removed from API call mt5_login_list


There should be no impact on existing applications.
